LIABILITY RELEASE FORMS
Liability release forms are required for all students participating in any off-site, school-related activities. The field trip form is for one time events only. The form must be completely filled out by a parent or guardian, signed, and brought to school before a student will be allowed to go on the trip.
No hand-written permission slips accepted.
No school slip, no field trip.
